It is our mission to provide you with affordable, high-quality treatment that fits your budget! We offer a variety of payment options to meet your needs. With benefits that last a lifetime, we understand that orthodontic treatment is a significant commitment on our patients’ parts. We are pleased to offer a variety of plans and policies, and ask that you review this information carefully.

Payment Responsibility

Patients/parents are responsible for all charges incurred through the office of Theresa Shaver Orthodontics, P.C.

If the insurance company does not pay any part of the amount expected, you will be required to pay the unpaid portion.
If you lose your coverage, you will be required to pay the unpaid portion.
If you have insurance coverage that provides you with a discounted treatment fee, and that coverage is terminated, your account will be prorated from the date of loss to reflect the balance of treatment fees at Dr. Shaver’s normal fee structure.
Patients whose accounts are 90 days or more past due may have their treatment terminated and accounts sent to a collections agency.

Payment Initiation

Fee assessment begins only when your active treatment program begins. If treatment is not recommended until a later date, Dr. Shaver will monitor tooth eruption and growth patterns during these important years, but you will incur no charges for any recall visits or subsequent X-rays until active treatment is started.

Payment Plans and Methods

Automatic deduction – Monthly payments can be deducted automatically from your bank debit or credit card each month, on the day of your choice.
Credit cards – We accept American Express, Discover, Visa and MasterCard.
Mail-in payments are accepted and are due on the first day of the month.
Interest-free payment plan – We offer a convenient interest-free payment plan based on the estimated length of treatment.

Discount Plans

A family discount may be available for one or more additional immediate family members who are undergoing, or have completed, orthodontic treatment at Theresa Shaver Orthodontics.
Multiple discounts for treatment (e.g., family discount and discounted insurance plans) are not available.

Insurance

If you have insurance coverage, we will file your claims for you once you provide proof of coverage and authorize payment directly to Theresa Shaver Orthodontics, P.C.
For discounted insurance plans in which Dr. Shaver is a provider, we will accept the plan’s fee. No other discounts will be provided (See “Discount Plans.”)
We will provide you with necessary receipts to submit for insurance/flex plans.

All About Insurance: Frequently Asked Questions (FAQs)

What do we need from you to file your insurance claims?

We’ll need to see, and make copies of your insurance card. We’ll also need the insured person’s name, contact information, birth date, social security number/subscriber ID#, and employer name/address.

How will my insurance company make its payment?

Most insurance companies make initial payments, followed by monthly or quarterly installments over the course of treatment. Payments are made directly to Theresa Shaver Orthodontics, P.C. We’ll work with your individual carrier to determine specific payment structure.

What happens if my employer changes insurance companies?

Should that happen you would need to contact our office as soon as possible. You’ll need to provide us with the new insurance company’s name, address and phone number so we can file a new claim.

What happens if I lose my insurance coverage?

Contact us immediately. The insurance company will no longer be sending payments on your behalf. You will be responsible for your remaining insurance balance.

What happens if I change jobs and get insurance from a different carrier?

Give us a call and let us know. We will work with you to determine if the new company provides a benefit, and if so, what that benefit is. We will then be happy to file with your new insurance company if you have orthodontic benefits.
